What does Half Hollow Hills School District need as documentation showing payments made?
Medicare not paid from Social Security Benefits
If you and/or your spouse are not receiving Social Security, Medicare bills directly on a quarterly or monthly basis. Copies of Medicare statements (or invoices) will satisfy the requirement for documentation of what was paid for Medicare Part B premiums. Half Hollow Hills only reimburses at the end of a calendar year, so please be sure to hold onto all statements (see below for sample). Since the paperwork and accounting is much more complicated, submit all 2023 documentation for you and/or your spouse at the same time using the the form provided (Click here for the form). If you are missing any statements, log into your Medicare account to print up to 12 months of payments. As a last resort, we will accept a print out of cancelled checks or electronic documentation - however, if you pay IRMAA or pay CMS late, reconciling your premiumus paid get very complicated and your reimbursement will be delayed.
Incomplete submissions will not be processed and a request for missing information will be made in writing. This will significantly delay your reimbursement payment.
