This site is intended to be a resource for Great Prairie AEA and LEA staff who support students in the area of Secondary Transition planning. On this site, you will find the GPAEA Transition Kit, the GPAEA Secondary Transition Bulletin, Learning Opportunities (updated regularly), and IEP Documentation resources.

The GPAEA Secondary Transition kit is comprised of: 

• Decision making process overview and tools (TR1)

• Diagnostic process (assessment) for living (TR2), learning (TR3), and working (TR4)

• Resources/tools for living (TR5), learning (TR6), working (TR7) 

• IEP documentation guidance (TR8)
