Scenario: Add a Contact Us form to your site to gather data from your site visitors. The completed form will look like this:
Create Form
Navigate to Site Manager > Pages and Content > Form & Questionnaire Builder
Click the Add a new Questionnaire button.
Input the following and press the Save Changes button:
Title of this questionnaire: Contact Us
Email address for answers: [as desired]
Form Style: White Background
Implement anti-SPAM: checked
Email Receipt Text for Users: Thank you for contacting training Lutheran Church. We will reply as soon as possible.
Include form data in email receipt?: Checked
Allow users to submit files with their form submission?: Unchecked
Add Form Data Fields
Click the form title to build and edit the form data fields.
For each question you wish to add to the form, click the Add Question button.
Click the link at the top of the page to view the completed form.
If desired, edit the top menu to add the link from the Forms tab.
