Scenario:  Add a Contact Us form to your site to gather data from your site visitors.  The completed form will look like this:

Navigate to Site Manager > Pages and Content > Form & Questionnaire Builder

Click the Add a new Questionnaire button.
Input the following and press the Save Changes button:
Title of this questionnaire:  Contact Us
Email address for answers:  [as desired]
Form Style:  White Background
Implement anti-SPAM: checked
Email Receipt Text for Users: Thank you for contacting training Lutheran Church. We will reply as soon as possible.
Include form data in email receipt?: Checked
Allow users to submit files with their form submission?: Unchecked

Edit Form

Add Form Data Fields

Click the form title to build and edit the form data fields.

For each question you wish to add to the form, click the Add Question button.
  1. Name: Single-Line Text type, not required, input box next to the question.

  2. E-mail address: Single-Line Text type, required, input box next to the question.

  3. Please contact me regarding: Drop-Down List type, required, input box next to the question.

    • After submitting the question, click the Add Answer Choice button to build the drop-down menu choices. 
      • Interested in membership
      • Questions about the Bible
      • Counseling need
      • Other
    • For each desired menu choice, type the choice and press the Submit button.  When you have added all of the desired choices, press the Back to Questionnaire link.

    • If desired, use the up and down arrows or Delete button next to each menu choice to reorder the display of the drop-down list options.


  4. Additional notes, needs or questions: Comment Field (Multi-Line Text) type, not required, input box next to the question.


Click the link at the top of the page to view the completed form.


If desired, edit the top menu to add the link from the Forms tab.
