Immunizations

New Students:

We are so excited to have your child/children at Mooneyham! Please forward the school a current shot record from your doctor as soon as possible so we can complete the registration process for your child. We are not able to place your child in a classroom or assign a teacher until the shots are current and up-to-date. Please see below for immunization requirements and resources.

Required Immunizations K- 12 grade:

Dtap- 4/5 doses, 1 dose after 4 years old
Polio- 3/4 doses, 1 dose after 4 years old
MMR- 2 doses, 1st dose after 1st birthday
Hep A- 2 doses, 1st dose after 1st birthday
Hep B- 2 doses, usually 1st dose at birth
Varicella- 2 doses,1st dose after 1st birthday or had the disease

All immunizations should be completed by the first date of attendance. A student may be enrolled provisionally if the student has an immunization record that indicates the student has received at least one dose of each specified age-appropriate vaccine required by this rule. To remain enrolled, student must complete required doses in each vaccine series on schedule and as rapidly as is medically feasible.

Students may be exempt from having immunizations for the following reasons: Medically contra-indicated( must have a physician statement that is only good for 1 year), Already had the disease( must have proof such as a titer drawn) and Conscientious objection. This requires a separate form which the parent gets from the Department of State Health Services. It has to be notarized and is only good for two years from the date of notarization.
