The mission of the DeForest Area School District’s School Counseling Program is to promote and enhance the life-long learning process by development in three broad areas:

Before filling out this form we would like you to think about a couple of actions you can take on your own to deal with a concern. 

*Did you talk to a teacher or an adult in the area?

*Did you use Name It, Claim It, Stop It to address the issue? 

*Did you talk to the person you were having an issue with? 

*Did you check Skyward? 

Please be aware that your counselor will connect with you as soon as possible. We do not want you to miss out on learning, so it will mainly be during lunch and recess. Also feel free to email your counselor if needed. Thank you!
