PRE-PARTICIPATION SPORTS EXAMINATION (PDF)

Student-athlete and parents/guardians review, fill-out and sign the front side of the physical form before the sports physical.

A physician will complete and sign the back of the form which is due every 395 days by the NCHSAA.

Physicals are due to Coach Reynolds before tryouts.

Completed by parents/guardians to give permission for student to participate in high school athletics.

Information Sheet and Concussion Statement Form (PDF)Reviewed and signed by the student-athlete and parents/guardians to permit for the student to participate.

Concussion forms are due to Coaches before the first game.
