Magnus Health Forms
Magnus Health is a new portal where parents can upload medical forms and information about their students. It will be used for the first time for the 2019-2020 school year.
Where to Access?
Parents can access the Magnus Health Portal by logging into MyDE, going to the Resources tab, and clicking on the Magnus Health tile.

How to Upload Forms
You can upload a photo of completed forms using your smartphone. Use the browser on your phone or an iPad (e.g. Safari or Chrome) to login to Magnus then follow the directions at How Do I Upload a Record to a Requirement.
If you are using your phone/iPad, you will have the opportunity to take a photo or pick from photos on your phone after you press the Choose File button:

Parents can reach out to the Magnus support Help Desk if they are having difficulty navigating the site, entering data online, or downloading forms.
