Name Changes

Hello CHS students! If you would like to have your name changed in the yearbook please fill out this form; it is due by October 31st . You must fill out this form to have your name changed AND have your legal guardian or school counselor email chs_yearbook@cusd.claremont.edu to have your name changed. All name changes that were made in previous years will not be automatically included; you must fill out this form again. Please keep in mind that after October 31st you cannot change your name.
