Application for Extracurricular Activity
All staff interested in overseeing a Schedule E club/activity should complete this application and submit to Principal or designee prior to the ILT meeting.
Extracurricular Activity Participation Report Form
All staff overseeing a club/activity should complete this online form to report participation numbers to be tracked by the district. Updated in 19-20 to include all needed items from the ROSTER report.
Extracurricular Activity ROSTER Report
All staff overseeing a club/activity should complete a roster to be stored on the school level. This should be shared with the high school athletic director and/or school principal.

SCHEDULE E CONTRACT REQUEST FORM
Administrators or Principal Designees must request a Schedule E Contract for all staff members after they have been approved by their ILT. Schedule E staff members do not request their own contract, they should work with their ILT and school staff to assess if their contract has been requested.
Principals/Administrators overseeing Schedule E should review their Administrator Manual to obtain the contract request form.

Clubs Requiring a PAP
Please note, in addition to all Athletic positions, any clubs with physical activity require staff members to have a Pupil Activity Permit.
