1. The best option:  Ask your clinician to send you a "New Insurance Information Intake Form".   It will automatically update our system to ensure that your insurance information is updated promptly.

2. The next-best option:  Use our confidential Insurance Update Form  to send us your insurance information securely.  Be sure to include a copy of the front and back of your card.
