Kindergarten Readiness
Listed below are some the forms that will be given at kindergarten roundup in April and will need to be completed before your child can be enrolled as a kindergartener in August. You will also need a certified copy of your child’s birth certificate, social security number, shot and immunization records, proof of residency (drivers license with correct address is fine), place of employment, emergency contact information, physician name and numbers, as well as any custody papers that might apply.
