The Supports Program and Community Care Program were developed by the New Jersey Department of Human Services’ Division of Developmental Disabilities (DDD), which provides public funding for certain services that assist eligible New Jersey adults with intellectual and developmental disabilities, age 21 and older, to live as independently as possible. What is the Purpose of this Guide? This guide summarizes the information in DDD’s Supports Program and Community Care Program Policies and Procedures Manuals – the rules that govern program eligibility and process – in a comprehensive, yet uncomplicated format for individuals and families. This guide is based on information contained in DDD’s Supports Program and Community Care Program Policies and Procedures Manuals. It is not intended to nor does it replace these policy manuals.

CTC Outreach Program

In 2015, Celebrate the Children started an Outreach Program designed to support First Responders interfacing with individuals with disabilities, particularly autism, in crisis or trauma situations. Our 1-2 hour presentation includes tips from an interdisciplinary team of Educators, Autism Experts, Occupational Therapists, Physical Therapists, and a Panel with special needs participants who can answer questions, and relate to their experiences. There is no cost for the presentation.
NJ Dept of Human Services 2020-2021 Resource Guide
We are excited to bring you the 2020-2021 New Jersey Resources Directory! The guide is a comprehensive resource identifying the various programs and services the Department of Human Services offers to New Jersey residents, caregivers and advocates. In this guide, a family can find out more information on how to apply for food, income and employment assistance, as well as health care and much more. A caregiver can get information on support programs and services to assist them in caring for their loved ones. And individuals with disabilities and their families can get information on various services that include in-home supports, vocational rehabilitation, and education. Voting: It's Your Right, a guide about voting rights and the voting process in New Jersey, was written for individuals with intellectual disabilities in clear language. It should also be of interest to anyone who would like to learn more about the legal and practical issues related to voting. The guide includes information about topics such as Who can vote in a government election?; What are you asked to decide when you register to vote?; How do you vote in person?; and How do you vote by mail? The guide is available in both English and Spanish. Voting: It’s Your Right was developed by The Boggs Center on Developmental Disabilities, Rutgers Robert Wood Johnson Medical School in collaboration with New Jersey’s Administration on Developmental Disabilities Partners, Disability Rights New Jersey and the NJ Council on Developmental Disabilities. It was funded by Disability Rights New Jersey’s Protection and Advocacy for Voter Access (PAVA) from the Administration on Developmental Disabilities, Administration on Children and Families, US Department of Health and Senior Services.

A Behavioral Wellness Toolkit
The purpose of this expanded version of the Toolkit is to empower individuals with disabilities and their family and professional caregivers by providing information to more effectively advocate for persons with complex severe behavioral health conditions (which we formerly referred to as “dual diagnosis'') for treatments, supports, services and the conditions that promote mental wellness. Individuals with diagnoses of developmental disabilities and behavioral health disorders face multiple challenges in their daily lives.
