CELL PHONE AND PERSONAL ELECTRONICS CLASS RULES
Students must keep electronic devices such as cell phones turned off and put away. Airpods, earbuds and headsets are not permitted in the school building. In accordance with school policy, failure to follow the county policy will result in disciplinary actions.
We will use a variety of educational, presentation, and online tools throughout the year.
School laptops will be provided and personal laptops can be brought in with permission as needed.
The smart phone, while a great asset to our lives, is a distraction in the classroom. Social media, selfies, games, texting, and listening to music are too tempting for most students to resist. Students need to be able to focus on learning and engaging with each other and me during a lesson without the distraction of their phones.
Parents: please review this policy with your child, and sign and return this letter the first week of school. I will post a copy of this letter to the class website for future reference.

Clinical Contract
EVANS HIGH SCHOOL CLINICAL CONTRACT 2023-2024
COLUMBIA COUNTY HEALTHCARE SCIENCE TECHNOLOGY EDUCATION
Name_______________________________________ Grade/Section_____________________
This contract is to be strictly observed at all times during your clinical rotation.
In the medical profession, a system of ethics exists that is observed and practiced by all medical personnel.
I will make it my responsibility to KNOW, UNDERSTAND, and ADHERE to the guidelines of this contract in order to assure the health and well-being of each patient.
Invariably the people with whom we deal are in different stages of ill health, which creates in each of them a different outlook. They become anxious, nervous, and quite often impatient. The afflicted require the best use of our technical and human-relations skills.
I will make every effort to be courteous, efficient, and accurate when helping patients through this stressful time in their lives.
Patients/Clients/Residents will often gain confidence in us and share their innermost thoughts. We also come to know a great deal about a patient from hearing reports and reading charts.
I understand that discussing:
Confidential information which includes patient or family history, diagnosis, prognosis, and treatment plan of a specifically named patient with other students, friends, or family is in violation of the “sacred trust of confidentiality” and the HIPAA law. Violations can result in large monetary fines or imprisonment. I will uphold the patient’s right to privacy.
(Cases will be discussed on a “need to know” basis for instructional enhancement only.)
County Dress Code for all clinical students in the medical office/hospital setting
I will wear:
The school approved uniform ONLY! It will be clean, neat & wrinkle-free. Undergarments must not be visible. Students can purchase the approved uniform for medical assisting from Scrubs of Evans during the specified time period only. Scrub pricing will be available at the beginning of the school year. I further understand that I am expected to wear the uniform the entire school day. I will not keep the uniform in my car, locker, etc.
The issued name badge/ ID with reel or clip at ALL times is an essential part of the uniform. Students may NOT attend clinical without this proper ID visible. All IDs will be attached to the uniform with the school approved badge reel.
Shoes for Allied Health must be clean solid black tennis shoes (all shoes must be approved by the instructor). Hair, natural colors only (no green, blue, pink, etc.). I DO NOT have permission to make stops of any kind on my way to or from clinical rotations. (Examples: Gas stations, restaurants, home)
I will not visit other units or departments without permission and purpose.
I will not leave the unit without checking with the person in charge. Out of courtesy, I will notify the person in charge before I leave the area. I will then report to one of the healthcare instructors if at the hospital.

If an accident/incident occurs while in clinical, I will report it immediately to my instructor and file the necessary incident report as directed.
I will respect and properly care for all equipment and supplies.
I will address ALL persons with the utmost respect. (Refer to #2)
I will complete and understand the “Self-Study” PRIOR to my assigned clinical rotations. If I am unable to do so, I will discuss this with my instructor. I understand that self-studies are done so that I will acquire a “Base Knowledge” of the assigned area.
I will collect information in order to complete a weekly “Learning Log”.
I will acquire an “Evaluation” at the end of each clinical rotation.
I intend to arrange the priorities in my life in order to make this a learning experience that will benefit me greatly in my future health career. I understand that assertiveness is appropriate, but aggressiveness is not appropriate. Self-motivation is important.
If I feel that I cannot meet the expectations of this contract, I will request a conference with my instructor immediately.
By signing this contract, I state that I am in good physical and sound mental condition to carry out all duties & assignments. TB tests and/or chest x-ray results, complete record of childhood immunizations, flu shot documentation, and COVID vaccination may need to be provided to the clinical instructor before attending any clinical rotations.
I understand that if I break this contract, disciplinary measures will be taken and expulsion from the program may occur. I understand that I remain under SCHOOL RULES/POLICIES during clinical and that all Columbia County Healthcare Science teachers have authority over me.
I DO NOT have any previous criminal history or school system infractions of which my instructor is unaware. Future infractions such as ISS / OSS will affect clinical class participation. Past or future criminal arrests of any kind make participation in the clinical program impossible.
Background checks will be conducted on each student at no cost by the Board of Education.
I understand that all clinical sites mandate a drug test on all persons using their facility. Drug test participation and the fee of the test is the responsibility of the student. Random drug testing may be done at any time during the school year at the students expense. I understand that if I fail a drug test that I will be removed from the clinical program and that further disciplinary action/criminal charges may occur.
