Student Health Forms 2025-2026

** ALL FORMS NEED TO BE UPLOADED IN MAGNUS HEALTH VIA FACTS 

ALL NEW STUDENTS: 

• Proof of Immunizations OR Exemption Letter

• Medication Authorization Form

• IF your child has a LIFE THREATENING Allergy: Food Allergy Action Plan OR Insect Allergy Action Plan 

• IF your child has Asthma: Asthma Action Plan 

• IF your child has a Seizure Disorder: Seizure Action Plan

• IF your child has Diabetes: Diabetes Action Plan 

• HIGH SCHOOL ONLY: Self Carry Epi- Pen Contract ,  Self Carry Inhaler Contract , and/or 

Self Carry Seizure Medication Contract

ATHLETES: 

• ALL athletes MUST have a CURRENT physical uploaded BEFORE tryouts.  Physicals are only uploaded to the athletics website.

• Forms will expire ONE YEAR from last signature date.

• NEW forms needed before start of school:

  • Immunization requirement for 7th and 12th grade students.  Meningococcal and Tdap vaccine at 7th grade. Meningococcal, 2nd dose, at  12th grade. 

  • Self Carry Contracts: Epi-Pen Self Carry Contract ,  Inhaler Self Carry Contract, Self Carry Seizure Medication Contract.
