REQUIRED FORMS & DOCUMENTS FOR REVIEW

When completing the online registration form, parents/guardians and student athletes will be required to verify that they have read and agreed to each of the following documents.  These documents may be printed out for reference only. DO NOT turn in these forms or documents.

• 1-BATAVIA HIGH SCHOOL CODE OF CONDUCT-LINK

• 2-IHSA SPORTS MEDICINE ACKNOWLEDGEMENT AND CONSENT FORM  -LINK

  • Parents/guardians and student athletes will electronically acknowledge this form on the registration document DO NOT PRINT THIS FORM OUT

    • Includes: concussion information, performance enhancing drug information and asthma medication consent.

  • If your son/daughter requires asthma medication (self-administered), the following must be done:

    • Print out the last page of the document

    • Complete the form and turn it in to the BHS athletic department

  • All athletes & parents are required to watch this video

• 4-BHS Athletic Department Meetings

  • • All Parents/Athletes must attend 1 seasonal sports open house meeting during 2022-2023
