Title IV Financial Aid Release Approval
If the above-named student is eligible for Title IV financial aid, e.g. Pell Grant, Stafford Loans, PLUS Loan, in excess of current allowable charges (such as tuition, fees, room, and meals) and the student wishes to have other charges (such as expenses like books, parking fine, health insurance, dorm fines, etc.) paid from the excess balance, the student must authorize Bethel University to pay these charges (see options below). Proceeds from financial aid may be used for prior year charges, not to exceed $200. A student may cancel or modify an authorization at any time. A cancellation or modification of this authorization is not retroactive; the cancellation or modification takes effect on the date that the University receives same from the student. If a student cancels an authorization to use FSA funds to pay for other allowable charges, the University may use FSA funds to pay any authorized charges incurred by the student before the notice was received.

CREDIT BALANCE
A Federal Student Aid (FSA) credit balance is created when the total of all FSA funds credited to a student’s account exceeds the total of tuition, fees, room, meals, and other eligible educational charges on a student’s account. Refunds are processed automatically within 14 calendar days of the date the student’s record indicates a credit balance, or within 14 calendar days after the first day of class if the credit balance was created before the first day of class (See Refund Policy on the University’s website).
DEFAULT ON ANY UNPAID BALANCE
I, the undersigned, agree to pay Bethel University, in full, all tuition and/or other charges incurred by me, and acknowledge that repayment of any such indebtedness shall be my sole responsibility, regardless of the sufficiency of the method of payment selected above to satisfy these charges. I understand that in the event of non-payment, I will lose the privilege of continuance in the program and my Bethel University transcript or grades will not be released until all outstanding indebtedness is satisfied in full. I further understand that the calendar for my selected programs of study will be set by Bethel University, and I agree to the terms and conditions set forth by the calendar and fee structure. I agree to pay Bethel University all attorney fees, court costs, and other collection costs and charges for the collection of indebtedness not paid when due. I further agree to reimburse Bethel University the fees of any collection agency, which may be based on a percentage of the indebtedness at a maximum of 33%, and all costs and expenses, including reasonable attorney’s fees, incurred in such collection efforts. Additionally, I agree that in the event that I withdraw, or lose student status, from Bethel University, the provisions for refund in the Bethel University addendum will be applicable and I will immediately pay any net amount that may be due to Bethel University and/or incurred collection costs at that time. I understand that any charges incurred during my enrollment period(s) at Bethel University, an I.R.C. §501(c)(3) not-for-profit organization, shall be considered an educational benefit and therefore non-dischargeable. Finally, I understand and agree that I will notify the Bethel University Billing Office of any address change(s). All payments and correspondence shall be addressed to Bethel University Business Office, 325 Cherry Avenue, McKenzie, TN 38201. The undersigned and Bethel University mutually agree that any dispute arising under or in connection with this agreement or related to any matter which is the subject of the agreement shall be subject to the exclusive jurisdiction of the courts located in Carroll County, Tennessee.
