All of our forms require a PPS Google account to access.

The Assistive Technology Team (AT Team) provides Trial Loans of assistive technology tools and materials for individual students.

Assistive Technology Tool Home Use

When an AT/AAC tool is checked out to a student on a Long-Term Loan, and it is determined that the student requires access to the tool at home for IEP goals or homework needs, the district and the parent/guardian must enter into a written agreement.

This document is intended to be printed, signed by the parent/guardian, then scanned and digitally sent to the AT Team before the tool starts leaving district property.

Privately Owned Equipment

”Privately owned equipment” means assistive technology equipment owned by the parent/guardian or assistive technology equipment not owned by the parent/guardian but provided by the parent for the student to use at school.

When privately owned equipment is being used at school, PPS and the parent/guardian must enter into an agreement about how the equipment will be used at school, how the parent/guardian can revoke consent, and how PPS will take reasonable precautions to protect the privately owned equipment.

Long-Term Loan Form

The Long-Term Loan(LTL) Form is provided when a student starts a new Trial Loan and can be found in their AT/AAC Google folder.  This folder is shared with the IEP team members or building's 504 coordinator.  A link to the folder will be emailed when the Trial Loan is ready for the student's use.

If a student's team would like to trial a tool that is available from building resources, email us at at@pps.net to have an AT/AAC Google Drive folder with LTL Form created and shared.
