Request Form

The attached form (below) is to request funding from the PTA. You must submit this form to treasurer Janelle Lochren in the PTA mailbox at least one week before the monthly meeting. Your attendance at the meeting is expected, as questions may arise that only you can answer. Meetings are held the first Wednesday of each month in the MS/HS at 6pm (see school calendar for any changes). Please be advised that the completion and submission of this form does not guarantee the request will be approved.
