Registration Checklist
Complete and signed registration packet
Copy of certified birth certificate
Copy of grade enrollment verification (current report card for grade verification)
Signed headsup concussion form
Signed CIF Concussion Return to Play (RTP) Protocol form
Signed and completed MVFL physical form (dated current season of registration)
These forms must be turned in and fees paid at a Sign-up event.
Fees for football and cheer
$250 Registration fee for football, jersey is included in price (fundraising fee due before 1st practice )
$100 Registration fee for cheer, plus the (fundraising fee due before 1st practice), cheer gear varies from year-to-year
$100 fundraising fee due before first practice, raffle booklet will be issued before first regular season game
