4 Island Aged Care was established in 2022 and through the Community Self Managed model we have been able to deliver efficient care services to the SMBI and Bribie Island Area to many happy clients with Home Care Packages.

Local workers are used to undertake your services.

You choose who, when and where they come from. You may know someone you would like to use.

Many collaborating opportunities come out of this Community Self-Managed model thereby strengthening our local community.

Your Community Support Coordinator is a local that you can contact to discuss and help sort any issues that may arise. ie help find someone to attend an appointment with you, source aids or equipment that can be delivered to your home. We will take the stress out of these decisions.
