Field Trip Form and School Resources

A Field Trip Ride Home with Parent Permission form must be completed and submitted to the classroom teacher PRIOR to the day of the field trip (Form available on the school website)
Parents are responsible for notifying their child's teacher when taking their child upon departure.
Parents are allowed to transport their child only.
