My 1994 PhD thesis is available here, including the 11th chapter that I decided to omit (I'm calling it chapter 9A as it would have followed the existing chapter 9). Various parts of the thesis made it into my 1998 book Dispositions but a lot didn't. Readers may find it interesting to compare.
