Field Trip Permission Slip

A single field trip permission form will be used for all field trips this school year. We complete one early in the school year which will be valid for all field trips. Here is a generic version that has some spots not filled in. (Generic Permission Slip Form)

Field Trip Chaperone Agreement

Volunteers may chaperone students on field trips providing that the chaperone has a Field Trip Chaperone Agreement on file with the school site. They must also complete the district mandated fingerprinting and TB process.

Fingerprinting

Please note that to volunteer in class or chaperone, you must be TB tested and finger printed. You may not volunteer or chaperone until Valle Verde is notified by the district that you are cleared. Fingerprinting form to complete is available here. Parents are responsible to reimburse the district $32.00 for fingerprinting. If you’ve already gone through the district’s fingerprinting and TB process, it will be noted in your file and you will not need to do it again.

Transportation of Students in Private Passenger Vehicles

Transportation to authorized events or field trips may be made in private automobiles. Prior to the field trip, an approved Transportation of Students in Privately Owned Vehicles form must be filed with the site administrator for each vehicle used. Drivers must be at least 23 years of age and must have adequate primary automobile liability coverage (see requirements listed on the form). Please be sure to complete one for this school year and to send in updated and current proof of insurance that shows the liability limits required.
