Reimbursement Form

Please fill out the reimbursement form and attach all receipts. Bring it all with you to our monthly meeting or email Serenity Aust to make arrangements for payments. Forms are also kept at CJH in the Booster Club box in the office.

(Reimbursements need to be completed within 30 days of transaction)
