Retake Request Form
From the handbook: If a student earns less than their desired score on a major test they may request to retake it. They will have 5 days from when the test was given back to request to retake, and are required to complete a request for retake form. They may be required to complete a remediation activity first. The recorded grade will be the higher grade.
