IF you already have a child enrolled in Franklin County Schools and need to add a NEW child, make sure you have the following information to turn in to the school:
Child’s Birth Certificate
Proof of Residence (mortgage, statement or current lease agreement in your name or a secondary document such as a current utility bill)
Proof of your child’s recent preventative health care exam
Up-to-date Kentucky immunization certificate
Proof of an eye exam
Proof of a dental exam
Court documents pertaining to your child, if applicable
Social Security Card
THEN, add the child(ren) when you complete your returning child’s OLR using the PARENT PORTAL LOGIN button above.
