Activity Log

According to National Health guidelines it is recommended that children aged 12-18 participate in at least 1 hour of moderate to vigorous physical activity 5-6 times a week. With this fact in mind we understand that Physical Education class alone does not fulfill this recommendation. Using Activity Logs helps students monitor the amount of physical activity they are getting outside of school to help meet this recommendation. These Activity Logs will count as homework for Physical Education class. Activity Logs will count as 10% of the student's written assessment grade and will be due every other Thursday.

The requirements for Activity Logs are as follows:

1. Student engages in at least 5 hours of physical activity over the 1 week

period.

2. Student writes down the type of activity they participated and how

much time spent participating in the activity.

3. Student must have a PARENT SIGNATURE on the Activity Log to

receive credit.

4. Student must turn in the Activity Log on the appropriate due date. Late

Activity Logs will be subject to the Middle School Late Assignment

policy found in the course syllabus.
