If, after reviewing your invoices and the appropriate back-up, you identify a charge(s) that you are questioning, please follow the steps below:
Complete the Telecommunications Billing Inquiry Form, provided below. Make sure all information requested on the form is provided.
Submit the form via email to OTTelcomDisputes@wv.gov
Once the form is received if not completed as requested, it will be returned to the agency for completion.
IS&C will notify the agency when an appropriate resolution has been found.
In order to help complete the form correctly, the following information should be included in the form:
The “Item” is the type of service being questioned: DAIN, Method One, VOIP, Data, SIP.
The “Nature of Inquiry” is the line number or circuit being questioned. Please list each line/circuit separately on the form.
The “Amount in Dispute” is the amount the agency is questioning.
Please provide the reason you are questioning this amount in the “Comments” section.
Please include the complete invoice that you received along with appropriate backup.
NOTE:
PLEASE BE AWARE THAT YOUR AGENCY IS RESPONSIBLE FOR PAYING THE FULL COST, ANY APPROVED DISPUTES WILL RESULT IN CREDIT ON A FUTURE INVOICE, NOT ON THE CURRENT INVOICE, ACCORDING TO WV STATE CODE §5a-7-4a(g).
