FEMA Individual Assistance Program
Assistance for Private Individuals and Households
FEMA Individual Assistance (IA) provides financial and direct services to eligible individuals and households affected by a disaster, who have uninsured or underinsured necessary expenses and serious needs. While FEMA IA does not cover businesses, we encourage businesses to fill out the survey. This information will be passed along to the Small Business Administration (SBA).
Scanning the QR Code to open the survey
1. Open the camera application on your mobile phone and position it directly over the QR Code (black square).
2. Once the image is focused, select the link. It will automatically send you to a website to fill out a survey.
3. Please fill out the survey to the best of your knowledge.
4. Once the survey has been submitted, the information you provided will populate to WV Emergency Management Division (WVEMD) database.
5. Please only submit one survey per household. WVEMD will reach out to you if any additional information is needed.
6. If you have neighbors in your community that may have issues accessing the survey, please assist them by completing the survey on their behalf.
