Medication In School
PLEASE BE AWARE THAT I CANNOT ADMINISTER, OR ALLOW STUDENTS TO ADMINISTER, ANY MEDICATION WITHOUT A DOCTOR'S ORDER. This includes Tylenol, Motrin, Inhalers, Benedryl. If your child will require any medications in school, please click one of the following links to retrieve the paperwork that needs to be filled out by his or her physician.
Note: Medications needed in school must be brought to the School Nurse in their original container and must match the doctor's order form. Medical orders must be renewed for each school year.

The New Jersey Department of Education mandates physical examinations for participants on school athletic teams or squads. The NJ Department of Education has updated the Student Athlete Pre-Participation Forms, which include the Health History, the Physical Examination Form, the Athletes with Disabilities Form: Supplement to the Athlete History, and the Medical Eligibility Form. These new forms are required to be used starting July 1, 2024. Also starting July 1, 2024, the additional athletic acknowledgment forms are to be completed by the parent/guardian in the Genesis Parent Portal.
Each student is required to have a medical examination conducted at the medical home of the student within 365 days from the first practice session/tryout
If the physical was completed more than 90 days from the first practice session/tryout, the Health History Update form must also be completed by the parent/guardian and submitted to the school
NEW - The Athletic Physical Examination Form and the Health History form are to be kept by the student’s healthcare provider.
NEW - The only part of the physical form that is to be returned to the school is the Medical Eligibility Form

Please make sure that forms are sent in to school at least 2 weeks prior to the 1st day of tryouts/practice to ensure that they are processed in time.
Middle School Students may utilize the following clinic for their athletic physical at no cost. You must bring the forms with you and inform them of which middle school your child attends.
Please note that the last day an 8th grade student can get a free physical is on March 3, 2026.
