Weber School District Health Screening Incentives 2024-2025
Employees
Current employees can choose between:
2 sick days to 1 PTO day
OR
$25 (to be paid on 03/01/2025 paycheck)
Retirees
Retirees will receive a $25 gift card
Spouses
Spouses of employees and retirees will receive a $25 gift card
NEW INCENTIVE THIS YEAR
Employees and Retirees who are enrolled in WSD medical insurance will also receive an additional $100 contribution to their HSA.
*If you and your spouse are current employees and are enrolled in WSD medical insurance, only one $100 HSA contribution will be given.
If you or your spouse were not able to make it to Weber School District's health screenings in November, you are still able to receive the incentives. Take the Physician Form with you to your healthcare physician and have them sign that you had a health screening. If you have already had a screening this year, you may use an Explanation of Benefits (EOB). Valid dates for an EOB are between February 1st, 2024 to January 31st, 2025. If you use an EOB, a Physician Form must be completed to show which incentive was chosen. Send the Physician Forms and EOBs by February 1st, 2025 to mcpalmer@wsd.net.

Health screenings or flu shots are not required by Weber School District but are highly encouraged.
