Massachusetts School Requirements 2021-2022

  • All Kindergarten Students must provide a copy of a recent physical, lead test and immunizations.

  • All 4th grade students must provide a copy of a recent physical.

  • All students should have a medical home and see their Primary Care Provider for physicals and medical concerns.

  • All students should have a dental home and see their Dentist every 6 months for cleaning.

  • All students should see their Eye care provider regularly or more frequently if needed.
