If you take a picture with a phone, either upload it directly to the drive from your phone, or email the picture to your WPS email address. From your email you can save the image to your Google Drive.

Answer each question to the best of your ability. Then click the "add file" button at the bottom of the form.

This will bring up folders you may have images stored in.
Finally pick "images"

This folder should have the image you saved from your Chromebook camera. Select the image and click open. The image should now be attached to the form!
