Accident Claim Procedure
In order to make a claim for insurance benefits the student must be sure he/she has taken the following steps:
1. Report accident to the faculty member in charge and the school nurse.
2. Report to the school nurse to secure an insurance claim form. An accident report must be on file.
3. Forward the claim to the doctor who was consulted as a result of the injury,
4. Request bill for services rendered (x-rays, etc.) and attach to claim form, if more than one doctor is involved,
5. Return completed medical form to the insurance company along with all final bills and x-ray reading reports attached, when the student is released by the doctor on the last visit.
All forms must be completed and returned to the school office not later than 90 days from the date of the accident. If claim cannot be completed by this time, an extension of time must be requested before the completion date.
Please note: This plan is a secondary, non-duplicating policy. If you have medical or dental insurance that covers your child, charges must be submitted to your primary insurance first.
