IMPORTANT HEALTH OFFICE NUMBERS

Our nursing staff is concerned with helping children attain the highest possible condition of well being. This is done by initiating measures of detection and follow through leading to correction of conditions that might impede physical or educational growth.

SCREENINGS

The School Nurse conducts annual vision, hearing, height and weight screening procedures. Convex vision screening, color perception and near point acuity is conducted at the Kindergarten level. Scoliosis screening is conducted for children in grades 4 through 5.

PHYSICAL EXAMINATIONS

Physical examinations are required for students entering kindergarten, second and fourth grades. A school physician will examine all students unless a record of a physical examination is presented by a family physician in accordance with New York State Education Department.

All kindergarten students must submit a report of examination prior to school starting. Students entering school as new entrants must submit a report of examination 14 days from the date starting school. Physicals must be conducted by a duly licensed physician who is authorized to practice medicine in New York State. Immunizations forms must be submitted before a student can be enrolled in school.

INJURIES

School Nurses are responsible for first aid care of school children who are injured or ill while under school supervision. The School Nurse is forbidden by law to apply or supervise a re-bandaging of any wound or to treat or evaluate a previous injury.

MEDICATIONS

Any student requiring medication during school hours must comply with the following procedure. A written order for prescription and/or non-prescription medication must be prescribed by an authorized licensed prescriber. A written statement from the parent or guardian requesting administration of the medication in school as ordered by the licensed prescriber is required. The parent or guardian must assume responsibility to have the medication delivered directly to the Health Office in a properly labeled original container.
