New Student Registration

Dear Parent/Guardian or Agency:

Thank you for you interest in having your child attend Honesdale High School.

Below is a list of documents required in order to enroll your child and complete a schedule.

1) Completed Registration Packet (Record Request is included in packet)

2) Copy of your child's birth certificate 

3) Copy of your child's immunization record and/or last physical*

4) Copy of Parent/Guardian's current residency (utility bill, lease or mortgage)

5) Copy of Parent/Guardian's current Driver's License.

*If you child is looking to play sports, please ask for a PIAA sports form and listing of sports with coach's contact information.

If you are a Guardian/Agency for this child, please provide a copy of the court appointed papers. Guardians will be provided with a Wayne Highlands School District Guardianship Affidavit, which will need to be completed, notarized and submitted with the registration papers.

Once we have your child's academic and health records, you will be notified and offered an opportunity to meet with your child's school counselor if necessary.
