What is Google Forms?
Use Google Forms to create online forms and surveys with multiple question types. Analyze results in real-time and from any device.
How does this benefit my students?
Students will benefit from the use of Google Forms as an assessment tool. Students, Staff and Families will benefit from it being used as a survey tool.
Design at least 3 surveys/quizzes you would like to create, ideally having the questions written prior to creating the form. If you plan to use any images/videos collect those materials.
Create at least 3 surveys or quizzes that you can use for your current position.
The following components need to be included in at least one of your forms:
Use of the template gallery
Use of videos
Use of images
Use of sections
Includes questions that go to a section based upon the answers of the person completing the form
Use quiz feature- including answer key and feedback features
Modify how responses are collected
Modify the presentation of the form (ex. Show progress bar, shuffle questions, have a submission response, etc)
Make at least one question required
Customize a theme
Text Style
Header image
Color
Background
Data from both the spreadsheet and charts in Google forms
Share form link
Collect responses
Analyze response data
After your lesson, reflect on your instruction using Google Forms and submit your evidence of mastery on this Reflection Form. Evidence of Mastery can be links to your 3 forms and a screen shot of at least one of the charts/spreadsheets of responses from your forms.
