What is Formative?
Formative is a web-based subscription that allows teachers to create digital formative assessments or assignments that are easily accessible from any device. The website allows users to create original assessments or upload pre-created documents/PDFs to embed questions to assess student learning.
Formative offer multiple interactive question types that allow learners to demonstrate their knowledge while using different types of embedded media to support all learners.

This is a MIDDLE SCHOOL-ONLY SUBSCRIPTION.
Determine what a topic you want formative data on and plan on your own or with an Instructional Technology Coach.
Build a minimum of THREE Formatives from scratch. Across all of your Formatives, you must include all the following features at least once:
At least 8 different question types
At least 2 content types
Add Standards: At least one question in each Formative should have a standard attached.
Assign the Formatives to your students in Google Classroom
Utilize the “View Responses” tab. Practice viewing each question separately by student and overall class. Try switching between classes.
Feedback: Give feedback to multiple students and show them how to access their comments.
Reports: Utilize the Tracker Tab on the Reports Page.

Sort by different criteria
After responding to student submissions, reflect on your instruction using Formative and submit your evidence of mastery on this Reflection Form.
Please include links to your Formatives. Other evidence of Mastery can be photos, links to lessons/presentations, student work, student data, videos of your lessons, etc.
