* Child’s Birth Certificate & Social Security Card * Parent’s Driver’s License or State ID
* Current Utility Bill with Parent’s Name & Address * Child’s Immunization Record
* Either Medicaid/SNAP proof or 2 Consecutive pay stubs * Other items required by sites
