• Office Visit Co-Pays. $30

• Deductible. HMO Plan $100 single / $200 family.

• Eligibility. Staff working 30 hours per week. Coverage begins date of hire or at annual open enrollment effective July 1st each year.

• Dependent Eligibility. Dependents are eligible to remain on the health plan through month-end they turn age 26.

• Open Enrollment. July 1st each year.

• Plan Options. HMO vs POS Side by Side Comparison. Staff enrolled in district insurance are eligible to change between the HMO or POS plans at renewal time effective July 1st each year.
