NEW SPORTS REGISTRATION PROGRAM: The Waterford athletic department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The program saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify that it is current and you will only be required to sign your forms once per school year (and after any update.)
Click on the Parent Playbook / Parent Playbook - Spanish to get started in FinalForms. If you require any support during the process, scroll to the bottom of the FinalForms website page and click "Get Help."
Register at FinalForms and follow the prompts to create your account, create your students and sign your forms.
PLEASE NOTE THE FOLLOWING IMPORTANT INFORMATION:
PARENTS are required to set up their FinalForms account and register their student-athlete in FinalForms with their email. Parents will receive an email prompting them to confirm and complete the registration.
Beginning with the 26-27 school year, parents/students will only need to sign their forms once per school year (and after any update). Parents will be able to register their student-athlete for fall, winter and spring sports simultaneously once registration opens in June 2026. This is especially convenient for multi-sport athletes who are currently required to register each season. 26.27 Sports Registration will open @ mid-August 2026. An email will be sent home announcing when registration opens.
A current physical examination is required to be on file with us BEFORE participation in any tryout, practice, or contest in interscholastic athletics at Clark Lane Middle School. In accordance with the policy of the Waterford Board of Education and the CIAC, the physical examination for sports is valid for 13 months. No athlete may legally participate until they have medical clearance (as verified by the school R.N.) and parental permission. Please contact the nurse's office at 860-443-2837 or emccaffrey@waterfordschools.org, if you have any questions or concerns.
Parents/Guardians are responsible for filling out this registration form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the nurse.

Thank you for expressing interest in joining the swim/dive team. The following information will hopefully answer any questions that you may have. This is our 10th season with a middle school swim team.
This is not a beginner’s swim lesson. If you have a sincere desire to learn how to swim competitively please see Coach Gathy, and he will give you some local options. Team members are expected to be able to swim at least 25 meters of freestyle. The team will have diving practice this season.
There will be 2-3 dual meets against other local middle schools in the area. We plan on swimming ELMS & HKMS during the season. There is also a CT Middle School Championship for those swimmers who make the qualifying times.
For practice, boys are encouraged to wear a suit called a jammer. Girls should wear a one-piece bathing suit. People with long hair are required to wear a bathing cap. Goggles and a towel should be packed from home. A water bottle will be a must for each practice. You have to stay HYDRATED!!
Team members will have the option of buying a team suit for our meets. Depending on the number of team members, a representative from Destination Athlete may visit us for an official sizing for our competition suits and team jackets. We will also be offering team members and their families customized CLMS Swim Team t-shirts.

__________
Unified Sports is a program that combines approximately equal numbers of athletes, with and without disabilities, on sports teams for training and competition. All unified sports players, both athletes and partners, are of similar age and matched sport skill ability. It was inspired by a simple principle; training together and playing together is a quick path to friendship and understanding.
CLMS sponsors a Unified Basketball program & we would be excited to have your child be a part of it as either an athlete or a peer partner. The commitment is approximately one (occasionally two) practice(s) a week, along with competitions between other schools a few times per season. Please have your child wear sneakers & comfortable clothes to practices and games.
