NEW SPORTS REGISTRATION PROGRAM: The Waterford athletic department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The program saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify that it is current and you will only be required to sign your forms once per school year (and after any update.)

Register at FinalForms and follow the prompts to create your account, create your students and sign your forms.
PLEASE NOTE THE FOLLOWING IMPORTANT INFORMATION:
PARENTS are required to set up their FinalForms account and register their student-athlete in FinalForms with their email. Parents will receive an email prompting them to confirm and complete the registration.
Beginning with the 26-27 school year, parents/students will only need to sign their forms once per school year (and after any update). Parents will be able to register their student-athlete for fall, winter and spring sports simultaneously once registration opens @ mid-August 2026. This is especially convenient for multi-sport athletes who are currently required to register each season.
A current physical examination is required to be on file with us BEFORE participation in any tryout, practice, or contest in interscholastic athletics at Clark Lane Middle School. In accordance with the policy of the Waterford Board of Education and the CIAC, the physical examination for sports is valid for 13 months. No athlete may legally participate until they have medical clearance (as verified by the school R.N.) and parental permission. Parents/Guardians are responsible for filling out this registration form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the nurse.

Sports registration opens @ mid-August. An email will be sent to all CLMS families announcing when registration opens. A follow up email will be sent home as a reminder to register for spring sports @ early February.
Volleyball and Track & Field are open to all 6th, 7th and 8th grade students.

2025-2026:
Registration is open to all 6th, 7th and 8th grade students. Please be aware that although boys may tryout/participate on the volleyball team, most contests are played vs. schools with all girl teams and in those instances, boys may or may not be able to participate in those games.
There are tryouts for volleyball and cuts will most likely be made. Please keep in mind, there is only space for 24 players.

2025-2026:
Track is open to all CLMS 6th, 7th and 8th grade students. The first practice begins @ the week before spring break and the regular season runs through approximately 5/31 with a championship meet in June for top athletes.
Based on previous registration numbers, there may be a cap placed on the total number of students allowed to register for the CLMS track team. This cap ensures that resources are appropriately allocated and that each participant receives adequate supervision and instruction. Priority will be given to 8th graders.
Please be aware that not all students may be able to attend away meets. Registration numbers and available resources will determine the feasibility of taking the entire team to away competitions. Priority may be given to students who have demonstrated commitment, skill or seniority within the program. These measures are implemented to ensure a fair and effective sports program while considering practical limitations.
Track practices are on Monday, Tuesday & Wednesday after school until 3:45pm and Friday after school until 4:30pm. Some days may change due to faculty meetings or track meets. Athletes can take the late bus home Monday-Wednesday or be picked up. On Friday’s, parent pick up is required. Athletes should be picked up by the athletic fields if not taking the late bus.
