NEW SPORTS REGISTRATION PROGRAM: The Waterford athletic department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The program saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify that it is current and you will only be required to sign your forms once per school year (and after any update.)
Click on the Parent Playbook / Parent Playbook - Spanish to get started in FinalForms. If you require any support during the process, scroll to the bottom of the FinalForms website page and click "Get Help."
Register at FinalForms and follow the prompts to create your account, create your students and sign your forms.
PLEASE NOTE THE FOLLOWING IMPORTANT INFORMATION:
PARENTS are required to set up their FinalForms account and register their student-athlete in FinalForms with their email. Parents will receive an email prompting them to confirm and complete the registration.
Beginning with the 26-27 school year, parents will only need to sign their forms once per school year (and after any update). Parents will be able to register their student-athlete for fall, winter and spring sports simultaneously once registration opens in August. This is especially convenient for multi-sport athletes who are currently required to register each season. Fall 26.27 Sports Registration will open in @ mid-August 2026. An email will be sent home announcing when registration opens.
A current physical examination is required to be on file with us BEFORE participation in any tryout, practice, or contest in interscholastic athletics at Clark Lane Middle School. In accordance with the policy of the Waterford Board of Education and the CIAC, the physical examination for sports is valid for 13 months. No athlete may legally participate until they have medical clearance (as verified by the school R.N.) and parental permission. Parents/Guardians are responsible for filling out this registration form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the nurse.

Sports registration opens @ mid-August. An email will be sent to all CLMS families announcing when registration opens. Cross Country is open to all 6th, 7th and 8th graders. Soccer is open to all 7th and 8th grade students.

2026-2027 Season:
First practice will be Tuesday, September 1st and run from 2:45 until 4:15pm. Parent pickup is required as late buses do not begin until September 9th. Beginning September 9th, those who are unable to get a parent ride may ride the late bus home at 3:45pm. Otherwise parent pickup will be M, T, W, F at 4:15pm (please note, there are NO late buses on Fridays). There are no tryouts for CC so any student that has signed up through FinalForms is welcome to join the team. Please make sure your athlete has all their medical info up to date and that all physicals are turned in to the nurse. The nurse will not allow students to participate if physicals are expired.
Athletes should have running clothes and shoes for practice. We will meet in the gym for practices. Based on previous registration numbers, there may be a cap placed on the total number of students allowed on the CLMS cross country team, and it is possible that not all students will be allowed to attend away meets. Registration numbers and available resources will determine the feasibility of taking the entire team.
Registration is open to all students in grades 6-8.

Open Assessments/Tryouts will take on September 3rd from 3:30pm-5pm and on September 4th from 3pm-4:30pm. Please bring a water bottle, athletic clothes, shin guards, cleats (no metal or cleated toe) and a good attitude. Specific practice days/times will be forthcoming from the coach.
Registration is open to all 7th and 8th grade students.
