Proof of birth (e.g., birth certificate, hospital record, baptismal record, passport, etc.)
Immunization Record
Proof of Residence (utility bill, lease, etc.)
Verification of Income (tax return, W2, DHHS statement, etc.)
Guardianship or Custody Documents (if applicable)

Program staff will contact you by phone and/or email to schedule your application appointment, complete your child’s application, and collect any additional documentation, if needed. If you change your phone number or email, please contact us to update your information.

Once you have completed the Steps 1-4, your child's application will be placed on the program's waitlist until Selections for Applications occur.

If one of your children has attended a Head Start or Great Start Readiness Program since 2013 or you receive a duplicate message, this means that you are already in our database system and we can update your records in our system. Please select the Online Application - Returning Family option below.
