Check your address to confirm you still live in the Plano Elementary school district. 

Login to your Infinite Campus Parent Portal. Once logged in, click on "More", then "Online Registration", then "New Student Registration" and complete the enrollment application. If you do not know your login, please contact Mrs. Beil at amber.beil@warren.kyschools.us for help.

Provide the school with the following documents:

• Two (2) forms of Proof of Residency documents (i.e., Utility Bill from the last 3 months, Lease Agreement, Copy of Deed, Voter Registration, Vehicle Registration, Notarized Statement of Residence, Military Orders).  

• Custody Documents (if applicable)

• Copy of Birth Certificate

• Current Immunization Certificate (must include Physician's signature)

• Initial Entry Physical Exam by a Physician 

• Vision Exam by Optometrist or Ophthalmologist 

• Dental Exam/Screening
