1. Birth certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC( and photocopy of BC issued by the Local Civil Registrar (LCR) duly authenticated by NSO and any supporting document indicating full name, date and place of birth, and citizenship.
2. Supporting documents are the following:
Voter’s affidavit or CTC of voter’s affidavit from the COMELEC;
Driver’s license;
National Bureau of Investigation (NBI) clearance (at least 2 years old); Old SSS E-1 form;
Baptismal certificate; Income Tax Return (ITR); School form 137 or Transcript of Record;
GSIS policy; Government Service Record: Seaman’s book;
Marriage contract;
Professional Regulations Commission (PRC) ID;
Postal ID;
3. Three (3) copies of the applicant’s new photo (colored with plain background) in 4.5cm x 3.5cm. Photo should show the applicant in decent attire with collar.
4. Personal appearance before the passport-issuing office by the applicant except when applicant is:
Not more than eight (8) years old; or,
At least 65 years old; or,
Mentally or physically incapacitated.
For applicants born on or before the year 1950:
A certificate of non-availability of birth record from the NSO may be submitted.
Joint birth affidavit signed and executed by two (2) disinterested persons.
Baptismal or CTC of voter’s affidavit from the COMELEC or any public document indicating date and place of birth and citizenship.
For applicants born after 1950:
In case of birth certificates with delayed registration, submit authenticated birth certificate and supporting document indicating date and place of birth and citizenship.
