Intellectual and Developmental Disabilities

Caring Hands Loving Hearts (CHLH) is a provider for Intellectual and Developmental Disability (IDD)services, through PA’s Office of Developmental Programs (ODP). Individuals who have a P/FDS, CLW or Consolidated waiver are eligible to receive the services that we provide. These services include:

1. In-Home and Community Supports (IHCS)

2. Companion

3. Respite

CHLH believes in taking a person-centered approach to the care that we deliver. We work closely with individuals, families, and collaborative care teams to develop care plans that fit the needs of everyone we serve – taking into account that each person we assist has their own talents, skills, interests and needs that are unique to them.

We train our Direct Support Professionals (DSPs) on the Individual Support Plan (ISP) that is written and maintained by the Supports Coordinator (SC). Furthermore, our team of Program Specialists and Directors ensure that the DSP understands the role that they play in the person’s Everyday Life.

Members of our team will participate in all meetings with Supports Coordinators and county agencies. We monitor each individual’s progress and advocate for changes in service(s) as necessary. We adhere to and follow the rules and regulations of PA ODP’s chapter 6100. Additionally, CHLH believes in and adheres to ODP’s Everyday Lives principles. CHLH understands that building trusting relationships are paramount to success. Therefore, we encourage friends and family members be hired with us to provide Direct Support Services for those with intellectual and developmental disabilities. We will provide training, support and oversight to any friend or family member providing services for their loved one.
