Download the 2024 - 2025 Pre-Approval Form.

You must have one form for each routine and title them accordingly.

Complete the form describing the skills as best you can, including the number of bases. 

Return the form in a Word “doc” format to ST Director at wsddja.st@gmail.com by midnight on Monday for a return by Thursday night. 

Attachments:

1. 1. Attach photos as necessary. 

   2. Attach music in an Mp3 format and include printed lyrics if possible. Google lyrics sites to check your songs. Note a specific part of your music if you have a concern.

   3. Attach any videos using a YouTube link. Title and number the skills. For example, say “this is hip hop skill #1", so it can be matched to your written form. Make sure your link has the correct settings that will allow the ST Director to access the video (i.e. unlisted link).

Form will be returned within a week.

If you have not received the form back within a week, contact the ST Director again as follow-up.

As you add and delete skills, send revised forms for approval. Use one form for each category. Please update form as you go rather than starting a new one for each time you send a new skill in for approval. We like to print them all for state to have on hand, and it is difficult when we have six different forms for the same routine.

Make and keep copies for competitions and give to ST judges prior to the event. If you want your copy back, pick it up when you pick up your ST penalty form after your performance.
