Embedded Files
If you click the icon in the top right-hand corner below,
it will pop the form out into a new browser window. There you'll find the option to:
download it under File -> Download.
Pick the format you want and fill in the copy
You do not need to request Edit Access
If you want to fill in the online version, go back to the main Entry form page
