This film follows Libby (Ashley Judd), a woman who is framed for her husband's murder. When she suspects he is still alive, she sets off on a search for answers. Tommy Lee Jones plays Travis, the parole officer who eventually sides with her as the truth is revealed. The majority of the film's scenes were set in Washington, but the filmmakers relied instead on locations in nearby British Columbia.
The lighthouse seen during the opening credits is located at Lighthouse Park at Point Atkinson in West Vancouver. It was also featured in the 2005 remake of The Fog.

The "Island County Courthouse" was actually the old courthouse in Revelstoke, BC - at 1124 Second Street West.
Prison scenes appear to have been filmed at Eastern Oregon Correctional Facility at 3405 Deer Park Drive SE in Salem, Oregon.
Library scenes were filmed at the Vancouver Public Library at 350 West Georgia Street in Vancouver, BC.
The ferry scenes were filmed on the Tri-Island Ferry between Port McNeill on Northern Vancouver Island, Alert Bay on Cormorant Island, and Sointula on Malcolm Island. I believe the dock seen briefly is in Sointula.

The scene of the car underwater was supposedly filmed at the John C. Stennis Space Center in Gainesville, MS (although that seems highly unlikely to me).
Farm scenes took place at 4769 112th Street in Delta, BC (thanks to Bruce H.).
The scene at the BMW dealership was filmed at Richmond Auto Mall in Richmond, BC (thanks to Yuji).
The town of "Evergreen, Colorado" was actually North Vancouver. The exterior of the art gallery is now Delany's Coffee House at 3099 Edgemont Boulevard. Libby pulls out of here in a hurry when Travis tracks her down.
Libby leaves Travis in the dust as she tears off heading SW on Edgemont through Crescentview Drive.

The hotel in New Orleans is the Hotel Monteleone at 214 Royal Street (thanks to Zach).
The cemetery was Lafayette Cemetery at 1400 Washington Boulevard in New Orleans. This was also seen in Interview With a Vampire.
